Assessing the "quality" of your practice

The current "trend" in healthcare is to assess providers for the "quality of medical care" they provide to their patients. ComChart Medical Software is committed to helping you analyze the quality of your practice, and staying "ahead of the curve." As shown below, you can create a list of the "Most recent result" for anything stored in ComChart. For example, you can create a list of all your patient's most recent LDL-cholesterols or the dates of the patient's most recent mammogram or pneumococcal vaccination. In the below example, a search of ComChart is being conducted for all patients who meet the follow criteria:

  • Age: 20 and 76 yo
  • Date of office visit: 4/1/05 to 9/30 /05
  • ICD code: 250.00 to 250.99
  • Labs test: LDL, HBA1c, urine microalbumin
  • Flowsheet: Diabetic foot exam, ophthalmic exam
  • Xrays: Thyroid u/s, mammogram, bone densitometry

The data generated looks like this and it can be exported to an Excel spreadsheet for further analysis.

DOQ-IT is the Doctor's Office Quality - Information Technology

DOQ-IT is the Doctor's Office Quality - Information Technology. This is a series of "quality assessment" indicators that you, your state licensing agencies and your patient's health insurance company are going to use to assess the "quality" of the medical care you provide to your patients. It is likely that there will be financial ramifications to the healthcare provider based upon the healthcare provider's DOQ-IT results.

In an attempt to help you "stay ahead of the curve," ComChart EMR has begun adding DOQ-IT modules to ComChart EMR so that you can do your own assessment of your practice, before the regulatory agencies and insurance companies do their assessment.

In addition to providing you with a list of patients who meet the DOQ-IT requirements, ComChart will also generate a list of patients who DO NOT meet DOQ-IT's requirements. Using this list of "non-DOQ-IT compliant patients," you have the opportunity to rectify the problem, thus resulting in better medical care and an improved DOQ-IT audit. The ability to generate a list of "non-DOQ-IT compliant patients" is a feature which most EMRs do not offer.

ComChart EMR v9 includes the DOQ-IT module for Diabetes. Additional DOQ-IT modules will be added to ComChart in the future. Below is a "screen shot" of the results of the ComChart DOQ-IT Diabetes assessment, displaying the type of data which is obtained by the DOQ-IT Diabetes analysis. In addition, a list of "non-DOQ-IT compliant patients" can be printed on request.

Currently, DOQ-IT has published DOQ-IT parameters for the following diseases:

      1. Coronary Artery Disease
      2. Diabetes
      3. Heart Failure
      4. Hypertension
      5. Preventive Care

The specific DOQ-IT measurement criteria, published to date, are as follows:

Coronary Artery Disease (CAD)
CAD-1: Anti platelet Therapy
Description: Percentage of patients with CAD who were prescribed anti platelet therapy

CAD-2: Drug Therapy for Lowering LDL Cholesterol
Description: Percentage of patients with CAD who were prescribed a lipid-lowering therapy

CAD-3: Beta-Blocker Therapy-Prior Myocardial Infarction (MI)
Description: Percentage of CAD patients with prior MI who were prescribed beta-blocker therapy

CAD-4: Blood Pressure
Description: Percentage of patients who had a blood pressure measurement during the last office
visit

CAD-5: Lipid Profile
Description: Percentage of patients receiving at least one lipid profile during the reporting year

CAD-6: LDL Cholesterol Level
Description: Percentage of patients with most recent LDL cholesterol < 130 mg/dl

CAD-7: ACE Inhibitor Therapy
Description: Percentage of patients with CAD who also have diabetes and/or LVSD who were
prescribed ACE inhibitor therapy


Hypertension (HTN)
HTN-1: Blood Pressure Screening
Description: Percentage of patient visits with blood pressure (BP) measurement recorded

HTN-2: Blood Pressure Control
Description: Percentage of patients with last BP < 140/90 mm Hg

HTN-3: Plan of Care
Description: Percentage of patient visits with either systolic blood pressure > 140 mm Hg or
diastolic blood pressure > 90 mm Hg with documented plan of care for hypertension


Heart Failure (HF)
HF-1: Left Ventricular Function (LVF) Assessment
Description: Percentage of patients with HF, who have quantitative or qualitative results of LVF
assessment recorded

HF-2: Left Ventricular Function (LVF) Testing
Description: Left ventricular ejection fraction testing during the current year for patients
hospitalized with a principal diagnosis of HF during the current year

HF-3: Weight Measurement
Description: Percentage of HF patient visits with weight measurement recorded

HF-4: Blood Pressure Screening
Description: Percentage of patient visits with blood pressure (BP) measurement recorded

HF-5: Patient Education
Description: Percentage of patients with HF who were provided with patient education on disease
management and health behavior changes during one or more visit(s) within a six-month period

HF-6: Beta-Blocker Therapy
Description: Percentage of patients with HF who also have LVSD who were prescribed beta-
blocker therapy

HF-7: ACE Inhibitor Therapy
Description: Percentage of patients with HF who also have LVSD who were prescribed ACE
inhibitor therapy

HF-8: Warfarin Therapy for Patients with Atrial Fibrillation
Description: Percentage of patients with HF who also have paroxysmal or chronic atrial fibrillation
who were prescribed warfarin therapy


Diabetes Mellitus (DM)
DM-1: HbA1c management
Description: Percentage of patients with one or more A1c test(s)

DM-2: HbA1c management control
Description: Percentage of patients with most recent A1c level > 9.0% (poor control)

DM-3: Blood Pressure Management
Description: Percentage of patients with most recent BP < 140/90 mm Hg

DM-4: Lipid Measurement
Description: Percentage of patients with at least one low-density lipoprotein (LDL) cholesterol test

DM-5: LDL Cholesterol Level
Description: Percentage of patients with most recent LDL cholesterol < 130 mg/dl

DM-6: Urine protein testing
Description: Percentage of patients with at least one test for microalbumin during the
measurement year, or who had evidence of medical attention for existing nephropathy (diagnosis of
nephropathy or documentation of microalbuminuria or albuminuria)

DM-7: Eye exam
Description: Percentage of patients who received a dilated eye exam or evaluation of retinal
photographs by an optometrist or ophthalmologist during the reporting year, or during the prior year
if patient is at low risk for retinopathy. A patient is considered low risk if all three of the following
criteria are met: (1) the patient is not taking insulin; (2) has an A1c < 8%; and (3) has no evidence
of retinopathy in the prior year

DM-8: Foot exam
Description: Percentage of eligible patients receiving at least one complete foot exam (visual
inspection, sensory exam with monofilament, and pulse exam)


Preventive Care (PC)
Blood Pressure Measurement
Description: Percentage of patient visits with blood pressure (BP) measurement recorded

Breast Cancer Screening Measurement
Description: Percentage of women 50-69 years who had a mammogram during the measurement
time period (24 months)

Colorectal Cancer Screening
Description: Percentage of patients screened for colorectal cancer during the one-year
measurement period

PC-7: Influenza Vaccination
Description: The percentage of patients 50 years and older who received an influenza vaccination
from September through February of the year prior to the measurement year

PC-8: Pneumonia Vaccination
Description: The percentage of patients 65 years and older who ever received a pneumococcal
vaccination

PC-9: Lipid Measurement
Description: Percentage of patients with at least one low-density lipoprotein (LDL) cholesterol test

PC-10: LDL Cholesterol Level
Description: Percentage of patients with most recent LDL cholesterol < 130 mg/dL

PC-11: Tobacco Use
Description: Percentage of patients who were queried about tobacco use one or more times
during the two-year measurement period

PC-12: Tobacco Cessation
Description: Percentage of patients identified as tobacco users who received cessation
intervention during the two-year measurement period
